વિધવા હોવા અંગેનું પ્રમાણપત્ર ગ્રામ પંચાય કચેરી ખાતેથી મેળવવાનું રહેશે

- રેશન કાડૅ
- લાઇટ બીલની ખરી નકલ.
- ટેલીફોન બીલની ખરી નકલ.
- ચુંટણી ઓળખ કાર્ડ ની નકલ
- પાસપોર્ટ ની ખરી નકલ
- First Page Of Bank PassBook/Cancelled Cheque
- Post Office Account Statement/Passbook
- Driving License
- Government Photo ID cards/ service photo identity card issued by PSU
- Water bill (not older than 3 months)
ઓળખાણનો પુરાવો (કોઇપણ એક)
- ચુંટણી ઓળખ કાર્ડ ની નકલ
- ઇન્કમટેક્ષ પાન કાર્ડની ખરી નકલ.
- પાસપોર્ટ ની ખરી નકલ
- Driving License
- Government Photo ID cards/ service photo identity card issued by PSU
- Any Government Document having citizen photo
- Photo ID issued by Recognized Educational Institution
Proof Needed In Service Attachment ( જરૂરી પુરાવા )
- Proof of age of eldest child ( Birth certificate / School leaving certificate / PAN card / Passport / Driving License )
- Pedhinamu/Ration Card to establish relationship of deceased with the applicant/Panchanamu
- Husband’s Death certificate or Panchnamu or Talati’s Recommendation
- આવક અંગેનુ પ્રમાણપત્ર
- ઊમર નો પુરાવો
